Please note that these Terms and Conditions do not cover the PI/PHI collected from offline resources and communications, except when the PI/PHI is combined with PI/PHI collected by us. They also do not cover third-party online resources to which our website may link, where we do not control the content or the privacy practices of the resources.
1. Age restriction
You must be at least 18 years of age to use this website, or any services contained within the website. Your access and use of the website indicates that you are at least 18 years of age. We assume no responsibility or liability for any misrepresentation of your age. We require a parent or guardian to communicate directly with us for all services related to children or minors.
2. Consent

Your access and continued use of the website will be deemed as consent. You may choose not to provide PI/PHI to us by electing not to enter any PI/PHI into a form or data field, and by not using any personalized services provided by our website or by any other online resources. However, some of the website may only be available to you if you provide PI/PHI or request the use of personalized services.
If you withdraw or limit your consent for us to collect or disclosure your PI/PHI, we may be limited in our ability to provide pharmacy services – as this may limit our ability to access essential information, or limit our ability to contact insurers, doctors, guardians etc. The continued use of our website will be deemed consent to obtain any PI and PHI that comes from your use of the website. Any PI/PHI collected or disclosed without consent will only be collected or disclosed in circumstances permitted by law. For example, where there is a concern for health and safety, or in an emergency.
Please note, certain PI and PHI will be collected automatically when you visit our website. If you have previously agreed to the use of your PI/PHI for marketing or promotional purposes, you may revoke your consent by following the opt-out instructions in each communication or by writing to our Chief Privacy Office (see address below). If you stop using the website, or access to the website is terminated, we may continue to use and disclose the PI and PHI in accordance with our Privacy Policy.
3. Protection of privacy
Through your use of the website, you acknowledge that when visiting the website or other online resources maintained by us, you may provide PI and/or PHI to us. By using our website, you consent to the collection, use, and disclosure of your PI/PHI in accordance with our Privacy Policy. If names and contact information for your physician and other health practitioners are provided via the website or via any other online resources, we may contact them to collect further information.

The information collected during this process includes, but is not limited to, (i) Internet Protocol (IP) address, (ii) Domain name, (iii) Browser type, (iv) Date and time of the request, (v) Information provided by tracking technologies, such as cookies and single-pixel tags (defined above), (vi) We may collect information about you from your mobile device, such as device ID and device type, usage information, use of mobile websites and other mobile resources if you are accessing the website from a mobile device.

We will take all reasonable and legally required steps to protect PI/PHI as it is transmitted from your computer to our website or other online resources and servers. We will also protect PI/PHI from unauthorized access, disclosure, alteration, or destruction. We ensure that all our affiliates and other third parties that are retained to perform services on our behalf and are provided with PI/PHI are contractually required to observe our Privacy Policy. However, security risks on the internet cannot be eliminated. It is your personal responsibility to secure your own copies of any logins, passwords and related access codes used on or for the website.
